   Here is a list of items you might be asked for during the claim process:
   Try to have as many as you can

Date of the occurrence
Type of loss or damage
Related injuries
Condition of your vehicle
Current mileage of your vehicle
Whether authorities such as police were notified
Vehicle license plate number of each vehicle involved
Copy of police report or police report number
Copy of any towing bills, medical bills, or any other bills
Name, address and telephone number of where your vehicle was towed
Names, addresses and telephone numbers of all persons involved
Names, addresses and telephone numbers of any potential witnesses
Drivers license number of the Operator, insurance company and policy number
Drivers license number of the Operator, insurance company and policy number
for the other driver, check the registration to see who owns the car
Accident description including:
- Where was the accident location – street address, cross streets
- Approximate speed of your vehicle
- Direction each vehicle was facing --
- If possible draw a sketch of accident
- What each vehicle was doing (stopping, turning, etc.)
- Description of accident scene
  (parking lot, intersection, highway, number of lanes, speed limit, signs, traffic lights)
- What were the weather conditions
